Waiver of Liability

I understand that the Honest Grappling Company LLC assumes no responsibility for injuries or illnesses, which I or my child may sustain as a result of her/his participation in any classes, athletic activities, sport programs, the use of any equipment, exercise or other activities. I expressly acknowledge that I assume the risk for any and all injuries and illnesses which may result from her/his participation in these activities. In consideration of the privilege of participating at Honest Grappling Company LLC, I hereby voluntarily release and discharge, its agents, servants and employees from any and all claims for injury, illness, death, loss or damage which I or my child/ren may suffer as a result of her/his participation in these activities. A parent/legal guardian must discuss with Honest Grappling Company LLC’s Instructor any special conditions or circumstances involving them or their child/ren. Special conditions must be discussed prior to enrollment.

I hereby give my permission to the medical personnel selected by the owner/manager on duty to order X-rays, routine tests, treatment; to release any records necessary for insurance purposes; and to provide or arrange necessary related transportation for me or my child. In the event that I cannot be reached in an emergency, I hereby give permission to the physician selected by the Facility to secure and administer treatment, including hospitalization for myself if I am unable or my child/ren. I understand that no accident or medical insurance is provided with any activity provided in the facility. I give permission to Honest Grappling Company LLC, without limitation or obligation, to use photographs, videos, film footage, or tape recordings, which may include my or child/ren’s image or voice for purposes or promoting or interpreting Honest Grappling Company LLC programs and release the company from any claim of liability to that use.
